Essential Pieces Family Support Services Inc. is a not- for- profit 501 (c) (3) Care Management Agency that is committed to connecting children and families to a continuum of community- based supports such as: Medicaid entitlements, Home and Community Based Services (HCBS), Respite, Community Habilitation and many other supports for children and families residing in communities throughout New York City, Nassau and Suffolk County.
Established in 2012, our agency is committed and takes pride in working with children and families on their journey to maintain safety, support and independence in the community.

Self-Directed and Habilitation Services
Essential Pieces Family Support Services, Inc. offers Self Directed and Habilitation services to individuals and families. Through Self- Directed and Habilitation services, teaching life skills to individuals served such as: travel training, activities of daily living, work readiness, budgetting, independent living and other skills all while providing families and caregivers support to manage other responsibilities.

Care Coordination and Advocacy
Essential Pieces Family Support Services Inc provides individuals and families with the resources needed to work with a Care Coordination and Advocacy program that will meet their individualized needs.
Care Coordination and advocay is provided by qualified care coordinators and uses a person-centered planning approach in developing, implementing, and maintaining an individualized service plan which is tailored to meet the needs of the person served. Care Coordination providers assist families with the review and updating of their Individualized Service Plan on an annual and semi-annual basis to ensure optimal care and service provision.

Angela has worked in the field of human services for over 25 years, and has received several awards in recognition of a successful track record in quality service provision and compliance. She takes pride in her work and her endless energy continues to keep her committed to providing service with excellence.

Program Enrollment Forms
New applicants will need to fill out the appropriate enrollment forms before services can begin. To make the process as smooth and efficient as possible, we provide the most important documents as downloads right here on our website.
Find out which forms you need to submit, select the appropriate download link and return the completed documents to our office either in person, by fax or as a scanned PDF file via e-mail. Naturally, all forms are also available at the office; however, filling them out in advance saves time.
